REGISTRATION INFORMATION

Registration Fee: $450.00

Registration: The complete meeting package is $450.00 (includes two continental breakfasts, two lunches, breaks, AV and other meeting/speaker expenses). 

Health and Safety: Up to date vaccination is required to attend the LeadingAge State Executive Forum. If you are not up to date* on your COVID-19 vaccinations, please take a PCR or antigen test to ensure a negative test result no more than 48 hours prior to arrival. 

* up to date with your COVID-19 vaccines is defined as when you have received all doses in the primary series and one booster when eligible.

 Meeting Schedule:

Monday, July 11: 8:30 am – 5:00 pm

Tuesday, July 12: 8:30am – 2:00 pm

 Monday July 11th Dinner: This dinner will be hosted by Jack York and It’s Never Too Late. More details to come!

Registration Cancellations: We realize emergencies come up, if you need to cancel your registration please submit your requests in writing to Sidrah Shayiq.

HOTEL INFORMATION

Hotel Accommodations: Members can make hotel reservations at The InterContinental Chicago Magnificent Mile here or by calling reservations at (877) 666 3243 and use the group code “XNJ” to get the group rate of $209/ night plus tax. Please make your hotel reservations by June 7, after that date we cannot guarantee a room or the group rate.
